Principal investigator Wendy Max, PhD, director of the Institute for Health & Aging, noted that from 2013 to 2018, e-cigarette use among high school students soared from 4.5% to 20.8%. The Australian Government’s new Public Health (Tobacco and Other Products) legislation commences from April 2024 and incorporates new controls governing the advertising and promotion of e‑cigarettes (Department of Health and Aged Care 2024). According to the National Tobacco Strategy 2023–2030 (Department of Health and Aged Care 2023b), strengthening regulations on e‑cigarettes has been listed as a priority area, and actions such as prohibiting the sale of flavoured e‑cigarettes have been proposed.
New 'pod-mod' style vape products are similar in shape and size to JUUL® and other vape sticks but are disposable. New York State has enacted strong policies to protect youth from accessing tobacco products and denormalize tobacco and nicotine use to prevent lifelong addiction, illness, and early death. Policies include banning the sale of flavored vapor products, ending price discounts on tobacco and vapor products, and banning the exterior display of tobacco and vapor product ads near schools. In regard to COVID-19 pandemic, the actual literature suggests that nicotine vaping may display adverse outcomes. Therefore, follow up studies are necessary to clarify the impact of e-cigarette consumption on human health in SARS-CoV-2 infection. Recently, a commonly commercialized crème brûlée-flavoured aerosol was found to contain high concentrations of benzoic acid (86.9 μg/puff), a well-established respiratory irritant [88]. When human lung epithelial cells (BEAS-2B and H292) were exposed to this aerosol for 1 h, a marked cytotoxicity was observed in BEAS-2B but not in H292 cells, 24 h later.

FDA regulates the manufacture, import, packaging, labeling, advertising, promotion, sale, and distribution of ENDS, including components and parts of ENDS but excluding accessories. An outbreak of EVALI in late 2019 and early 2020 put thousands of people in the hospital. Since then, EVALI cases have been declining, but people who vape can still get EVALI. Research from the CDC shows that vaping among youth has declined somewhat since 2020.
Still, many people, including teens and young adults drawn to vaping, may not realize that, Bittoni said. Other popular brands, like Puff Bar, have received warnings from regulators about the sale of flavored devices as well. Following the Golden State, Washington had the second-lowest amount of youth vapers, at eight percent. In the Evergreen state, the legal age to purchase vapes is 21, and there is a ban on all flavored varieties. Last year, Alaska Senate President Gary Stevens, a Republican, introduced Senate Bill 89, aimed at imposing the state's first-ever tax on e-cigarette products. Wyoming had the highest rate of youth vapers, with 30 percent of kids under 18 reporting they had used e-cigarettes in 2023.
In response to this issue, the PACT Act Amendment prohibits sellers from using the U.S. Post Office to ship e-cigarettes, vapes, flavored and smokeless tobacco products. The PACT Act applies to all shipments in interstate commerce (15 U.S.C. § 376) as well as to all delivery sales. Some people who smoke choose to use both cigarettes and e-cigarettes at the same time on an ongoing basis, whether they are trying to quit or not.
Researchers noted that they were unable to assess any association between e-cigarette nicotine formulation and cigarette cessation outcomes. Another potential limitation was that the study only included adult participants who reported smoking within the previous 30 days, limiting information about younger smokers and former smokers. In that same vein, 5.9 million people began vaping, bringing the total use of tobacco products among those 12 and older to 64.4 million.
Almost 9 in 10 people (86%) supported prohibiting the sale of e‑cigarettes to people under 18 years of age, a substantial increase from 2019 (79%). Social and economic factors shape people’s behaviours of vaping or smoking. Generally, people living in the most disadvantaged socioeconomic areas were the most likely to currently smoke regular cigarettes but not use e‑cigarettes (13.2%). In contrast, people living in the most advantaged socioeconomic areas were the most likely to use e‑cigarettes but not smoke regular cigarettes (6.6%) (Figure 4). Young people aged 18 to 24 were the most likely to have used e‑cigarettes in 2022–2023, with almost 1 in 2 (49%) having used them at least once in their lifetime, and over 1 in 5 (21%) currently using e‑cigarettes in 2022–2023. The agency says companies were blocked because they couldn’t show the possible benefits for adult smokers outweighed the risk of underage use. The companies say they had prepared detailed plans to avoid appealing to young people. In general, most of the cytokines detected in BALF were significantly increased in WT mice exposed to PG with nicotine compared to PG alone or air control [42]. Some of these effects were found to be through nicotine activation of NF-κB signalling albeit in females but not in males. In addition, PG with nicotine caused increased macrophage and CD4+/CD8+ T-lymphocytes cell counts in BALF compared to air control, but these effects were ameliorated when animals were sub-chronically exposed to PG alone [42]. Researchers know more about the long-term effects of smoking than those of vaping. However, vaping products contain a great deal fewer harmful chemicals than cigarettes and can be an effective tool in quitting smoking altogether.

Smokers differ in their needs and I would advise them not to give up on e-cigarettes if they do not like the first one they try. It may take some experimentation with different products and e-liquids to find the right one. An expert independent evidence review published today by Public Health England (PHE) concludes that e-cigarettes are significantly less harmful to health than tobacco and have the potential to help smokers quit smoking.
A 2019 randomized control study also found that daily e-cigarette use leads to an almost doubled rate of smoking abstinence than other nicotine-replacement products after one year. Vaping involves breathing in an aerosol that contains several chemicals, including nicotine and flavoring, through an e-cigarette or vape. Such policies are critical in the absence of strong federal regulation of e-cigarettes. As of March 31, 2023, seven states and an estimated 388 counties, cities, towns, and tribes have restricted the sale of flavored tobacco products. The harms of e-cigarette use in young people include not only the deleterious effects of nicotine, but also exposure of the lungs and airways to potentially toxic solvents and flavoring chemicals. The rapidly rising rates of use in young people and the high rates of daily use strongly suggest that many are addicted to nicotine and will have difficulty in stopping use of all tobacco products. One study found that people who use only e-cigarettes increase their risk of developing lung disease by about 30% compared with nonusers. Electronic cigarettes (e-cigarettes) and other vapor products are devices used to heat and aerosolize (turn into particles) highly addictive nicotine or other substances that a person can inhale into their lungs.

When studying tobacco cigarettes, researchers rely on smoking machines that simulate how frequently a typical smoker takes a puff and how much smoke is inhaled with each breath. No one has yet determined how much e-cig vapor the typical user breathes in, so different studies assume different amounts of vapor as their standard, making it difficult to compare their results. The prognosis depends on how much nicotine was taken and how quickly treatment was started. If a person is able to survive during the first four hours after poisoning, they’re usually likely to recover. If a person has been severely affected, they may have ongoing seizures or respiratory failure or other problems because of the damage done from low oxygen levels during the nicotine overdose event. Increased levels of nicotine or cotinine (nicotine metabolite) can be detected in urine or blood. In fact, the overall evidence points to e-cigarettes actually helping people to give up smoking tobacco. There is no evidence that e-cigarettes are undermining England’s falling smoking rates. Instead the evidence consistently finds that e-cigarettes are another tool for stopping smoking and in my view smokers should try vaping and vapers should stop smoking entirely. E-cigarettes not only pose substantial health risks to youth and young adults, they pose a significant environmental threat (see the Truth Initiative fact sheet on Tobacco and the Environment). Almost half (49.1%) of young people don’t know what to do with used e-cigarette pods and disposable devices. A second brand of e-cigarettes—marketed as Nixotine, Nixodine, Nixamide and Nic-Safe—contained a nicotine analog called nicotinamide, also at levels lower than the labels indicated, and combined with undisclosed amounts of 6-methyl nicotine.

Despite these uncertainties, the use of e-cigarettes has skyrocketed since their introduction about a decade ago, particularly among young people. The Food and Drug Administration estimates that more than 3.5 million middle and high school students used e-cigarettes in 2018, though sales to minors are prohibited. For example, in an analysis using primary human umbilical vein endothelial cells (HUVEC) exposed to 11 commercially-available vapours, 5 were found to be acutely cytotoxic, and only 3 of those contained nicotine [24]. Despite the general decline in smoking, it remains more prevalent among youth with lower socio-economic position (SEP). This has been observed with family-level SEP indicators, including parental education [3, 4] and family affluence [5]as well as individual-level indicators of adulthood SEP such as academic performance [4, 5] and vocational education [6–8]. For smokeless tobacco (snus) use and electronic cigarette (e-cigarette) use, the associations with SEP have been mixed [7, 9, 10]. It can’t hurt to talk with a doctor or other healthcare professional about the risks of vaping, especially if you already have a chronic health condition, such as asthma. More research needs to be done in order to understand the side effects of nicotine-free vaping. Some of these cellular changes have been linked to the development of cancer over the long term, though there’s currently no evidence to suggest that vaping causes cancer. The 2018 NAP report found substantial evidence that vaping causes cell dysfunction, oxidative stress, and damage to DNA.
The association between student- and school-level factors and susceptibility to smoking. Nationally, e-cigarette use among high school students doubled from 11.7% in 2017 to 27.5% in 2019. The results showed that people who used e-cigarettes at any point were 19% more likely to develop heart failure compared with people who had never used e-cigarettes.
